STATEMENT OF THE AGENCY:
Commonpoint is a non-profit, multi service agency with flagship sites in Little Neck, Bayside, Forest Hills, East Elmhurst, Jamaica and over 70 additional sites throughout the community that provides a diverse array of programs and services to over 70,000 mission is to sustain and enhance the quality of family and individual life for all members of our community, regardless of religion, ethnic or cultural background, national origin, gender identity or expression, sexual preference, age, or disability, within a pervasive and inclusive environment that facilitates individual growth, meets individual needs and builds community. Department Overview:
Commonpoint High School & College Success consists of youth employment and school-based wraparound support to adolescents and young adults ages 14-24. The HS&CS department provides access to work readiness training, social-emotional learning, individual/group counseling, college access and academic support, connections to internships for individuals who are motivated to enter the workforce. The High School and College Success division provides community programming at the Youth Opportunity Hub, located in Jamaica, Queens.

STATEMENT OF THE POSITION:
Within our framework of a holistic, strength-focused, and family-supported practice, an individual hired as a Bilingual Case Manager will report directly to the Site Supervisor at Queens Academy High School. The positions will take place in person at Queens Academy High School, 138-11 35 Avenue, Flushing NY 11354.

Qualifications:
  • Bachelor's Degree required in a related field.
  • Fluent in Spanish or Mandarin.
  • At least 3 year's experience working with youth in an urban setting.
  • Astute knowledge of resource development, case management and client advocacy strategies.
  • Experience working with students on achieving positive outcomes for success.
  • Comprehensive case coordination experience required.

Responsibilities:
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Cultivate a collaborative effort to provide services to young adults within Queens Academy High School to ensure successful outcomes and barrier reductions.
  • Handle case assignments, draft service strategies, plans and regularly review case progress.
  • Document case progress, referrals and services with thorough case notes.
  • Work collaboratively with the internal teams of both Commonpoint and DOE staff within Queens Academy High School, based on the priorities set forth by School Administration.
  • Develop and maintain a network of resources and partnerships to address needs and concerns of clients.
  • Navigation and liaise between programs and services to ensure seamless service delivery.
  • Case conference and regular communication with school and agency staff.
  • Establish a strong and appropriate rapport with all students. Assist them in reducing barriers to receiving services.
  • Lead, participate and coordinate events and campaigns, as defined by school administration and the Commonpoint site supervisor.
  • Participation in staff meetings, program meetings, and staff development in order to support client progress.
  • Track data and efficacy of all internal and external referrals and provide metrics and data analysis for program partners and stakeholders.
  • Navigate concerns and issues related to language barriers of students and families.
  • Facilitate Work based learning Geared Workshops to Program participants.
  • Complete other tasks as assigned with flexibility.

Job Details:
  • Salary: $60,000-$62,500 with education and experience
  • Hours: Full-Time, 35 hours per week

  • Expected Start Date: September, 2026

General Tasks of All Commonpoint Staff:
  • Promote and implement the Core Values of the Commonpoint and NYC Department of Education.
  • Participation in all staff meetings and staff training seminars.
  • Participation in agency-wide programs and program committees, as assigned.
  • Active involvement, as a supervisee, in the supervision process.
  • Support the mission of the Commonpoint
  • Other tasks appropriate to the Commonpoint staff, as assigned.
